These services offer an individualized planning approach while keeping the person’s health and safety in the forefront. Eligibility and type of assistance varies by service.
Service Coordination – provides advocacy, linkage, and referral services that allow individuals to live as independently and productively as possible.
Traumatic Brain Injury (TBI) Services – provides supports and services to people diagnosed with traumatic brain injury (TBI) who are at least 18 years old.
Nursing Home Alternatives – provides supports and services to those who would typically require nursing home placement to maintain their health and safety.
Home Care Services – provides home care supports and services. A Registered Nurse provides oversight to all orders from participant’s medical practitioner recommending oversight, supervision, personal care, mobility, meal preparation, and/or light housekeeping.
Family Support Services – provides recreational activities, information and referral services, and reimbursement for goods and services related to the child’s disability to families who have children with disabilities living at home.
OPWDD Waiver Services – assists with daily living skills, activity programs, and environmental modifications in the consumer’s home.
