Home Care Services
Home and Community Support Services are provided through Pathways, Inc. Licensed Home Care Services (LHCSA). A Registered Nurse will provide oversight to all orders from the participant’s medical practitioner recommending oversight, supervision, personal care services, mobility, meal preparation, and or light housekeeping tasks. Services are paid for through Medicaid or can be privately paid.
The following Services are offered through both the Traumatic Brain Injury (TBI) Waiver and the NHTD Waiver.
- Home and Community Support Services
- Independent Living Skills Training and Development
- Respite Care
- Structured Day Program
