Nature Of Work:
• The Individualized Care Coordinator (ICC) assists the family in identifying its strengths and needs, while coordinating care through a network of providers and the county/counties’ system of care. The ICC monitors delivery of services and supports with the child and family through service plan/budget development and review. In addition to providing the individualized care coordination service detailed below, he/she may deliver clinical services as needed and appropriate for each family.
• Education: A Bachelor’s Degree in Social Work, Psychology, Human Services or related field required. Masters Degree strongly preferred.
• Experience: With a Bachelor’s degree, a minimum of four (4) years of experience in providing services to children and families with Mental Health Difficulties is required. With a Master’s degree, a minimum of two (2) years of experience in providing services to children and families with Mental Health Difficulties is required.
• Must meet the requirements of the School Age Care, Day Care, and/or Mental Health Regulations regarding a State Central Register Clearance regarding history of abuse/maltreatment of children and Criminal History Review for criminal convictions.
