Pay: $19.00 - $21.50 per hour! (depending on related experience)
Nature of Work:
The Habilitation Coordinator will attend ISP meetings and write plans for individuals in the Community Hab and Employment programs. Will train staff to implement said plans. Maintains compliance with all pertinent regulations, policies and procedures and agency philosophy. Supervises program staff in the absence of the Program Director to ensure quality services are provided, including training, scheduling, communication and service implementation. Assigns mentor, monitors completion of training and paperwork. Works cooperatively with Behavior Intervention Specialist, Medicaid Service Coordinators, families and residences to assure consumer services are provided. Educates and encourages each individual, staff, family member and/or other providers to the best of their ability by offering choice, respect and quality services. This may include working as an Individual/Family Directed Option staff member, whose schedule and training is provided directly by the individual or family, in collaboration with Pathways, Inc.

Minimum Qualifications:
- Associate's degree from an accredited program in the human services or related field, three (3) years’ work experience with developmentally disabled individuals and one (1) year supervisory experience preferred OR a High School diploma, five (5) years’ work experience with developmentally disabled individuals and two (2) years supervisory experience preferred.
- A valid driver’s license passing insurability guidelines.
- Must meet the requirements related to Criminal History Record checks that became effective April 1, 2005, which requires prospective employees to be fingerprinted if they have regular and substantial unsupervised or unrestricted physical contact with people receiving services in our programs regulated by OPWDD.
