High Fidelity Wraparound is a national, evidence-based process that focuses on the goals of the family to address complex emotional and behavioral needs of the youth. The High Fidelity Wraparound Care Manager partners with the family and facilitates the intensive process with the collaborative team, including natural supports and community providers, while honoring family voice. The team builds the family-specific strengths to get needs met, learn new skills, and develop skills and supports to manage crisis.

Population and Geographic Area Served

High Fidelity Wraparound (HFW) serves youth ages 6 to 21 who reside in Chemung, Schuyler, Steuben, or Yates County, have a diagnosed Serious Emotional Disturbance or two mental health conditions, are involved in multiple child-serving systems, and have recent or imminent risk of disruption of living situation due to behavioral needs. The youth and family must be willing to actively participate in the intensive process throughout its four phases. All youth eligible for High Fidelity Wraparound must be Medicaid eligible.

Access to Services

You can contact us at any time to find out if your child/youth is eligible for assessment for High Fidelity Wraparound. Your child/youth can also be referred through the children's Single Point of Accessibility (SPOA) or Yates County school district staff. Your child cannot be assessed without your involvement.
