This level of care works with families to empower and engage them in treatment so that the youth, when appropriate, may return home upon discharge.

The Lake Breeze CR program is licensed by the New York State Office of Mental Health.

Population and Geographic Area Served

The Lake Breeze Community Residence (CR) program serves children in the Finger Lakes, Southern Tier, and Rochester, New York regions between the ages of 13 and 18 years with serious and persistent symptoms of cognitive, affective, and personality disorders. 

Access to Services

All referrals must be approved by the county SPOA committee. Referral packets for admission to the Lake Breeze CR may be obtained by contacting the Program Director. Completed referrals will be reviewed by a multidisciplinary Referral Review Committee to determine eligibility.
