Nature of Work:
Works with the Registered Nurse to ensure the provision of all nursing related services for the residential site. Provides information and input as a member of the Service Planning Team. Maintains communication with the manager to adequately meet nursing needs and to keep the manager well informed of the medical status of each individual. Assists the Registered Nurse in assuring compliance with all applicable regulatory standards and policies and procedures of the agency. Also is responsible for the actual implementation of each consumers Individualized Program Plan as determined by the consumer and his/her Service Planning Team. This includes but is not limited to; ongoing teaching/assisting with community integration, social and recreational activities, money management, transportation, behavioral needs and overall health and well being of the consumer.
Minimum Qualifications:
- High School Diploma or GED is required. Must be licensed and currently registered by New York State Education Department Office of the Professions as an LPN. Must have at least one year specialized training or one year of experience working with individuals with ID/DD.
- Must meet the requirements related to Criminal History Record checks that became effective April 1, 2005 which requires prospective employees to be fingerprinted if they have regular and substantial unsupervised or unrestricted physical contact with people receiving services in our programs regulated by OPWDD.
