Pay: $22.17-$22.51
Nature of Work:
A Service Coordinator acts as an advocate and liaison for individuals with Traumatic Brain Injury (TBI) or disabilities, or senior citizens, by honoring the individual’s choices while assisting in accessing services within their community. The Service Coordinator facilitates assessment, linkage, referral, and monitors the needed or desired service, as it applies to each person.
Minimum Qualifications:
The Service Coordinator must either:
- be a licensed social worker, hold graduate degree in social work or psychology, master of gerontology, licensed physical therapist, registered professional nurse, certified teacher of students with disabilities, certified rehabilitation counselor, licensed speech pathologist, or licensed occupational therapist, and 1 year of related experience, OR
- have a bachelors degree and 3 years of related experience, OR
- have an associates degree and 5 years of related experience.
